Requests for admissions is similar to interrogs (my sample is on another hard drive that crashed):

PLAINTIFF'S FIRST SET OF REQUESTS FOR ADMISSIONS TO DEFENDANT

Plaintiff, pursuant to Rule 36 of the Federal Rules of Civil Procedure, requests that defendant, so and so and so and so., within thirty (30) days after service of these requests, make the following admissions for the purpose of this action:

REQUEST FOR ADMISSION NO. 1:
Admit that you lied you scum bag collector on such and such date at so and so time.

RESPONSE:

REQUEST FOR ADMISSION NO. 2:
admit that you are not licensed in the state of so and so

RESPONSE:

REQUEST FOR ADMISSION NO. 3:
admit that you placed a trade line on the plaintiff's credit report on such and such date.

RESPONSE:

and so on and so on....
it all depends on the facts that you know about your case...
what have they lied about?
what state laws have they violated
when did they call you
what did they say in the phone conversation
