The ones I have filed were laid out like a pleading but entitled, "Plaintiff's Notice of Pretrial Readiness" (Defendent's if you are the Defendent) and noted such things as motions, exhibits, and witness lists. It also included a statement of undisputed facts. You should find out what the court requires for going to trial, make a list of the items done and not done and get what you need to get done completed for the notice. The notice basically lays out for the judge what will be presented. Check your RCP's for instructions on what to include. Call the court clerk as well and see if any local rules apply.
