There's the age thing mentioned by steve g. Also, just like other financial products, prepaid companies are required to verify your identity to comply with federal laws and statues regarding money laundering, terrorists, etc... So if you've moved recently or changed names (married, divorced), or just young and haven't established a "public" record, you might not have passed the identity verification screen.
Or, the company could have just messed up.
If you're at least 18, try readydebit.com. I have a card and think it's a good prepaid Visa. Lots of options for loading the card (direct deposit, cash, bank account, Paypal) and low fees.
